Hi everyone, I am [Your Name]. In this video, I will briefly discuss my practicum experience, my work on the capstone project and changes throughout the RN-to-BSN program. This experience has not been easy but it has been worthwhile as it has helped to refine me as a professional and reaffirmed my passion towards patient care. I can attest that managing patients with chronic conditions has given me practical experience, and on designing my capstone project, I enhanced a good understanding of how to develop effective interventions according to research and best practices. Overall, the program has enriched my professional knowledge and reignited my commitment on enhancing the quality of our patients’ care with kind and knowledgeable approach.
Practicum Experience with Heart Disease Patients
Based on my practicum experience, my clients were middle-aged and older adults living with heart disease. This population is particularly challenged by issues of adjustment to lifestyle modifications, medication regimen, and overall symptom control. My position enabled me to assist such patients with regard to self-care measures, which are vital in the management of the disease. The possibility to observe the positive changes in patient-related outcomes was really gratifying, for this evident result of patients’ targeted educational programs can enhance the work of people, who are focused on the patient self-management and continual quality improvement.
Intervention Development and Impact
The intervention that was formulated for my capstone project revolved around the aspects of diet, exercise, medication regimen, as well as symptoms management with an emphasis on heart health. To foster patient involvement and self-management, I incorporated wearables into my practice, allowing patients to track important health indices in real time. It was not only useful in establishing a prognosis but also empowered the patient by offering them a means to monitor their initial steps towards recovery. Self-administered educational handouts completed simple added to the intervention by providing clear instructions on the behavior of the patients hence adapting to heart-healthy behavior. These perceptions were fully supported by nurse-facilitated structured help with processing information and experiences that fostered greater perceived control and important roles in adherence.
Patients and their families also supported the intervention highlighting that most of them felt more empowered to manage their health issues. Family involvement was also significant as it offered a strong framework of support outside of the clinical environment or even supporting vital changes at the familial level. The support from relatives helped patient to maintain positive attitude and, in case of chronic disease, to adhere to treatment, which often requires much patience.
