Care coordination is primarily essential since patients' needs, plans, goals, and care plans pass through diverse phases and sometimes different workers, hence avoiding the loopholes that may occur and compromise patients' care. One of the most detrimental issues with various kinds of healthcare providers is that there often seems to be a lack of communication between them and their respective patients; care coordination eliminates or at least dramatically reduces these holes in information exchange, thus making a much more integrated and cohesive experience for patients. For example, when physicians, specialists, and hospital staff have an open line of sight to the patient's particulars, patients get more appropriate follow-ups and prevent readmissions. They also avoid repeating specific tests or developing different treatment plans that may be damaging. In other words, more significant coordination benefits patients as the population becomes happier with the healthcare system and has a generally improved health outcome.
